Open an Account > How to Open a New CAMP Account as a Participant

1.Read the Program Guide/Information Statement.

2.Pass a Resolution/ Ordinance to join the JPA.

3.Submit a copy of the Resolution, completed Account Application , signed Declaration of Trust, and a completed IRS Form W-9.
